Two Clalit Research Institute team projects deemed best practice models by the Commonwealth Fund

On September 8th, 2017, as a part of the Commonwealth Fund’s International Experts Working Group on Patients with Complex Needs, Professor Ran Balicer of the Clalit Research Institute co-authored the Ten Recommendations for Policymakers in which they describe best practices for providing health care to patients with complex needs.

Furthermore, 2 of the 12 international profiles deemed best care models for patients with complex needs were Clalit projects:  the Comprehensive Care for Multimorbid Adults Program (CC–MAP) and the Preadmission Readmission Detection Model (PREADM).  Both projects were led by Professor Efrat Shadmi and Calanit Kay of Clalit Health Services with analytics and predictive modeling work done by the Clalit Research Institute team.
